12-step treatment in Jaipur

What do you work? a guy in my Jaipur hotel asks. I'm an addiction therapist, I say. A counsellor? Yes. Oh, I'm a drug addict, he says and adds: NA. I know what NA is all about, I'm a twelve-step therapist, I smile. I went to therapy to a treatment nearby, he continues. I still go there from time to time. To remind me that I do not want to spend time there again. Would you mind showing me the center? I inquire. Not at all, he says. And so I get to see my first Indian treatment center and have a talk with the people there, all of them former addicts and volunteers. I learn that only a small minority come into treatment out of free will , that most are brought here by their families. And what is your success rate? 20 percent, I'm told. If that is true - and I have no reason to not believe them (they are not in it for the money) - that is not only impressive but higher than the rate of treatment centers that insist their clients must be undergoing treatment voluntarily.

AA in der Praxis

Was die AA-Literatur sagt, ist das eine, wie AA in der Praxis funktioniert das andere. Obwohl AA-Meetings weltweit nach dem selben Schema ablaufen (wobei es ganz verschiedenartige Meetings gibt: von Schritte-Meetings zu Diskussionsmeetings, von offenen zu geschlossenen etc.), sind es die Teilnehmer, welche letztlich bestimmen, wie diese Meetings ablaufen und es sind auch die Teilnehmer, von welchen die Stimmung im Meeting abhängt (und es ist oft die Stimmung - und gar nicht so sehr das Programm - , die darüber entscheidet, ob jemand bei AA bleibt oder eben nicht). Ich habe von Meetings auf den Philippinen und in Italien gehört, bei denen angeblich Alkohol getrunken worden sein soll, war in Meetings, die ich schwer erträglich fand im Sinne, den David Foster Wallace (1995) beschrieben hat: "So then at forty-six years of age I came here to learn to live by clichés ... To turn my will and life over to the care of clichés. One day at a time. Easy does it. First things first. Ask for help. Thy will not mine be done. It works if you work it. Grow or go. Keep coming back", und ich war in Meetings, die ich wegen der dort gehörten persönlichen Geschichten als hilfreich und bereichernd empfand. Anders gesagt: welche AA meinen wir eigentlich, wenn wir fragen, ob AA ein wirksames Programm ist?

Genesen mit AA?

Da es offenbar so viele Interpretationen von AA wie AAs gibt, und da man nicht wissen kann, wie AA individuell praktiziert wird, ist es fraglich, ob es überhaupt möglich ist, AA in einer Art und Weise zu überprüfen, die gängigen wissenschaftlichen Standards entspricht. Trotz des Fehlens eines wissenschaftlichen Nachweises, glauben viele - begründet durch persönliche Erfahrung - dass AA wirksam ist. Dass alle, die das glauben, durch eine Gehirnwäsche durchgegangen sind, ist unwahrscheinlich (und wäre auch schwierig nachzuweisen), doch auch wenn sie es wären: die Tatsache, dass sie soffen bevor sie zu AA kamen und dann aufhörten, legt den Schluss nahe, dass ihre Trockenheit etwas mit AA zu tun hat.

Ursache-Wirkungs-Methodologien scheinen nicht geeignet die Komplexität von Suchtverhalten und - behandlung zu erklären, da sie die Widersprüche und Paradoxien, die Teil menschlichen Verhaltens sind, nicht zu erklären vermögen. Sie können auch Kernfaktoren wie Motivation oder Glauben, die wesentlich sind für die Genesung, nicht messen. Nicht zuletzt aus diesen Gründen ist AAs act yourself into a new way of thinking den Behandlungen, die auf die Identifizierung von Ursache und Wirkung ausgerichtet sind, vorzuziehen.

Künftige Forschung und Behandlung müssen neue Wege finden "the non-logical aspects" (die Wunder, Widersprüche und Paradoxien) der Sucht anzugehen und die Ursache-Wirkungs-Methodologie anzupassen, zu ergänzen und möglicherweise zu überdenken.

Drugs & Crime

Drug-related offences are dominated, on the one hand, by income generating non-violent property crime to support drug use, and, on the other hand, by violence associated with drug distribution. There seems to be agreement that there is a link between dependent drug use and crime but that there is no single, causal connection between drug use and crime which means that present policy is demonstrably based on un-evidenced assumptions. A devastating verdict, especially in light of Nutt's (2009) recent comment: "It is very easy to get research money to show that drugs are harmful but it’s very hard to get research funds to show that they may not be so"

So how come that, despite the lack of concrete evidence for a drug-crime link, the established approach is partly based on the argument that there is such a link? One reason is that institutions and policies once in place are difficult to change, the probably more important reason however is that drugs are essentially criminalised for moral reasons. In the words of James Q. Wilson "drug use is wrong because it is immoral and it is immoral because it enslaves the mind and destroys the soul". Or for other reasons (political, cultural etc) that do not respond well to hard factual evidence.

Alcohol Abuse in Russia

Per capita consumption of alcohol in Russia is among the top ten highest in the world, and the problem continues to grow. The Balashov Narcological Clinic in the Saratov region is one of many scattered all over Russia to treat alcoholics. Many patients have already experienced the terrible torture of delirium tremens but cannot stop drinking, as very often entire families drink habitually, from generation to generation.
